Having ready access to sample OSHA forms is critical to an effective Occupational Safety and Health (OSH) compliance plan. For example, OSHA requires certain employers to prepare and maintain records of work-related injuries and illnesses. Employers must keep accurate records so they can effectively administer company safety and health programs. There are specific OSHA forms for many safety recordkeeping requirements.

A Look at Each OSHA Form
Employers who are not exempt from OSHA's recordkeeping requirements must prepare and maintain records of work-related injuries and illnesses. Here is a look at each OSHA form:

  • The Log of Work-Related Injuries and Illnesses is OSHA Form 300. This OSHA form is used to classify work-related injuries and illnesses and to note the extent and severity of each case
  • The Summary is OSHA Form 300A. It shows the totals for the year in each category. At the end of the year, employers must post this OSHA form in a visible location from February 1 to April 30 of each year
  • The Injury and Illness Report, OSHA Form 301, is used to record supplementary information about recordable cases
Employers should record on OSHA Form 300 those work-related injuries and illnesses that result in death, loss of consciousness, days away from work, restricted work activity or job transfer, or medical treatment beyond first aid. Employers must also record on the OSHA form work-related injuries and illnesses that are classified as "significant" or meet certain criteria.

Employers must also record any significant work-related injuries and illness that is diagnosed by a physician or other licensed health care professional. Employers must keep a Log on site for each establishment.
