This is a model template prepared by OSHA to comply with the agency’s COVID-19 Emergency Temporary Standard (ETS) on Vaccination and Testing. The ETS remains mired in litigation and it seems unlikely it will be implemented. However, for organizations choosing to implement a mandatory COVID-19 vaccination policy, this model a template created by OSHA may be a valuable resource.
